School Prep

Before sending your child back to school, make sure you have these steps done:

• Make sure you have your child’s Diabetes Medical Management Plan (DMMP) from their endocrinologist
• Get in contact with the school nurse, your child's teachers, and administrators to make sure all forms are complete and everyone is ready to help your child succeed
• Get your child’s diabetes supplies together so they are ready to monitor their blood sugar and administer insulin/glucagon
